01 — In office

Hands-on, comprehensive care.

Office visits are appropriate when a physical examination, procedure, testing or face-to-face assessment is important. The clinic is intended to serve as the central home for adult primary care.

02 — Video or telephone

Convenience with clinical boundaries.

Virtual visits may support medication follow-up, results, stable-condition check-ins, Lifestyle Medicine and care planning. Availability will depend on location, consent, payer rules and clinical appropriateness.

03 — Residential settings

Primary care where eligible residents live.

For selected AFHs and senior communities, Loza plans to provide in-person care in the resident’s familiar setting while coordinating with staff, family and outside clinicians.
