Founded by Yirgalem Tiruneh, MD—known by many patients and community members as Dr. Yirgu—and Tingrit Desta, PA-C, Loza Health brings together physician leadership, broad clinical experience and a commitment to accessible, culturally responsive care.

The practice is being designed for adults across the lifespan—from people strengthening prevention and everyday health to older adults managing multiple conditions and transitions in care.

A familiar clinic setting for comprehensive examinations, preventive visits, chronic-condition care, procedures within scope and concerns that require hands-on evaluation.

Video or telephone care

Planned virtual follow-up for clinically appropriate concerns, medication review, results and care planning. Some conditions will still require an in-person, urgent-care or emergency evaluation.

AFH & senior-setting visits

In-person visits for eligible residents where they live, with coordination among the resident, authorized family or representative, residential-care team and outside clinicians.

Virtual and residential services remain subject to final launch availability, geographic coverage, consent, payer rules and clinical appropriateness.

Your first clinical visit will focus on your history, priorities, medications, current concerns and the preventive or chronic-care needs that deserve attention.

You and the clinician will agree on appropriate next steps, which may include testing, treatment, referrals, follow-up or a longer-term care plan.

Planned video or telephone visits will complement—not replace—hands-on primary care. Whenever possible, the same Loza care team will use the patient’s established history and care plan to support continuity.

Loza Health is developing a coordinated primary-care pathway for eligible residents of Adult Family Homes and selected senior communities. The model is designed to support routine medical care, chronic-condition management, medication review and collaboration with authorized families and the resident’s broader care team.

Understand who is providing care, how authorized family members may participate, what happens after a visit and which concerns belong with Loza, the residential team, urgent care or 911.

For AFH and senior-care teams

Use a defined process for enrollment, visit preparation, medication and record availability, routine questions, changes in condition, orders and follow-up. Final communication standards will be published before service begins.

For clinicians and discharge teams

Confirm setting, location and service eligibility before sending records. A secure referral route and required-document checklist will be available before referrals open.

Planned specialized service

Medical care after a motor vehicle injury.

Loza Health plans to offer timely, non-emergency medical evaluation and follow-up for adults after motor vehicle collisions. Care may include:

  • Initial history and examination after the injury
  • Assessment of pain, mobility and functional limitations
  • Appropriate imaging, therapy or specialist referrals
  • Medication review and conservative treatment planning
  • Follow-up of symptoms, recovery and return to usual activities
  • Clear medical documentation of evaluation, treatment and clinical progress

Dr. Yirgu brings more than 20 years of clinical experience spanning inpatient hospital medicine, adult primary care, chronic-disease management, healthcare leadership, Lifestyle Medicine and healthy-aging care. He is board-certified in Internal Medicine and Lifestyle Medicine, a Fellow of the American College of Physicians and a Blue Zones® Certified Physician.

Tingrit Desta, PA-C

Tingrit is a University of Washington–trained physician assistant with more than 18 years of healthcare experience across public health, hands-on caregiving, behavioral health, primary care and senior care. Her background supports accessible, coordinated and compassionate care for adults and eligible residential-care patients.

Insurance & payment options

More than one route to care.

Insurance

Loza Health intends to participate with Medicare and many major commercial insurance plans. The confirmed plan list will be posted after credentialing and contracts are effective.

Self-pay visits

For people without insurance—or those choosing not to bill insurance—a transparent flat rate per visit is planned. Prices and included services will be published before scheduling opens.

Membership option

A membership pathway is being evaluated for predictable access and pricing. Final benefits, exclusions and any insurance relationship will be reviewed before it is offered.

Coverage and payment depend on the individual plan and service. Insurance participation does not guarantee that every service is covered.
